Why We Exist
The YC Foundation serves the Greater Houston area with home attendant services, housekeeping, shopping, cooking, and medication monitoring. Our Case Management Program coordinates home care, home-delivered meals, and other essential services for physically and mentally frail residents.
The need we see
Independence fades, needs increase
Conditions such as Alzheimer’s and Parkinson’s reduce mobility, memory, and daily function. Without help, safety and nutrition decline and small problems become emergencies.
Families carry heavy loads
Caregivers trade health, income, and time to keep loved ones at home. Burnout is common and isolation makes it worse. A clear plan and reliable support change everything.
Help exists but is hard to reach
Services are scattered across agencies and forms. Case management and warm handoffs keep seniors from falling through the cracks.
